The body of a south London man has been flown back to Britain with two hearts and four lungs.
The extra organs were found stitched in a plastic bag inside Louis Selo, 55, who died on holiday in Ireland on August 2.
Mr Selo, a father-of-three from New Malden, suffered a massive heart attack just after getting into a taxi at Dublin Airport with his wife Anna and their teenage daughter.
An initial post mortem was carried out at Beaumont Hospital, Dublin, before the body was sent back for his family to bury him.
Only then a second examination on home soil - a routine step when Britons die abroad - uncovered the organs.
A major inquiry is underway at Beaumont Hospital. "It could lead to disciplinary action," a hospital spokesman said.
The institution, which performs about 300 post mortems a year, has since traced the donor of the extra heart and lungs.
The organs has been returned to the donor's family for burial.
"What happened should not have happened and we express our deep regret to those affected," the spokesman said.
"The hospital got in touch with both families the moment we became aware of the situation."
Mr Selo, a former Standard Chartered Bank worker, was in the media five years ago when he flew his son Sebastian to Australia for rare brain surgery.
